Why you may be hearing from Carelon about palliative care
You may have received a call or referral through your health plan. Your plan works with Carelon to identify members who could benefit from additional palliative care services to better manage symptoms and coordinate care.
Carelon partners with your health plan to provide this support as part of your covered benefits. It is designed to support you, not replace your current care. Participation is always your choice.
