Our palliative care team
The Carelon palliative care team is a physician-led, interdisciplinary group of health care providers who help improve the quality of life for patients living with serious illness.
At Carelon Palliative Care we help manage patients’ symptoms and relieve pain, while offering emotional support to patients and their families.
We dedicate ourselves to enhancing patients’ quality of life because we believe everyone deserves comfort and dignity during life’s most challenging times. Our whole health approach, which focuses on integrating the physical, social, and emotional aspects of patient care, makes a significant impact by easing patients' suffering and empowering families with guidance and support.
We support patients and their loved ones throughout their care journey, helping them feel heard, understood, and cared for.
Teamwork is the only way we work
Doctors know their patients’ health best. Led by a palliative care physician and joined by nurses, social workers, and support teams, Carelon delivers care at home to people living with serious illness, following their existing doctor's treatment plan. We work closely with patients and their caregivers, primary care providers, and specialists to provide coordinated support. When it comes to helping patients live life their way, teamwork is the only way we work.
Role of palliative care team members
Carelon’s physician-led palliative care team includes a palliative care doctor, an advanced practice provider, a registered nurse, a palliative care social worker, a patient engagement specialist, and a patient care coordinator.
Advanced practice provider (APP)
An advanced practice provider delivers comprehensive palliative care by diagnosing and managing patients’ conditions and providing specialized support to help improve patient outcomes.
Palliative care doctor
The palliative care doctor specializes in managing patients’ symptoms, helping relieve pain and providing emotional support to patients and their families.
Palliative care social worker
The palliative care social worker offers emotional support, counseling, and practical assistance to patients and their families. This helps them navigate their health care journey, make informed decisions, and cope with the challenges they face.
Patient engagement specialist
The palliative care specialist focuses on strengthening patient engagement, enhancing communication, and educating patients and their families. This helps better ensure the patients’ needs and preferences are understood throughout their care journey.
Patient care coordinator
A palliative care coordinator organizes and streamlines care delivery, ensures effective communication among health care providers, and supports patients and their families in navigating the complexities of managing serious illnesses.
Registered nurse
The palliative care nurse provides compassionate support by managing patients’ symptoms, administering treatments, and offering emotional care to patients and their families.
Conditions that the palliative care team manages
Our palliative care team manages a variety of serious and complex conditions, including cancer, heart disease, lung disease, kidney failure, neurological disorders such as Alzheimer's and Parkinson's disease, in addition to other chronic illnesses.
How the Carelon palliative care team collaborates with your health care providers
Carelon’s palliative care team works closely with your health care providers to help you receive coordinated, comprehensive care. This collaboration involves regular communication and consultation with the doctors, nurses, and specialists who deliver your treatment to integrate palliative care into your overall care plan.
Our team shares expert insights on managing symptoms such as pain, nausea, and fatigue, and addresses psychosocial and emotional needs, contributing to a whole health approach to care. By aligning treatment goals with your values and wishes, we help avoid unnecessary interventions, reduce hospitalizations, and support smoother transitions between different care settings.
This collaborative approach helps keep your health care providers aligned, leading to improved coordination and a more personalized and supportive care experience for both you and your family.

“My nurse practitioner was terrific. She did everything and probably kept me out of the hospital a few times. It just changed everything. I felt better. I knew they’d be there. They gave me a magnet [with a 24/7 support number] to put on my refrigerator and said to call anytime, seven days a week, around the clock."
– Margaret
“It is wonderful to have home visits. It has relieved a lot of anxiety to know we can call whenever we need you.”

“After my cancer diagnosis…the discomforts, the pain, my whole thought process, my memory…it's hard to describe. My nurse practitioner told me to call at any time no matter what it is — no matter how small or how big. And I can only imagine what it would've been like not to have had these guys.”
– Ron
“If a patient has access to a smartphone, tablet, or computer, virtual care is something that allows us to be more available to them. A lot of patients have really embraced it.”

“My mom was able to stay at home and out of the hospital during her last few weeks, which were our family’s wishes. Mom was even able to dance at her grandson’s wedding. She really moved her feet — she didn’t just shuffle. That’s how we’ll remember her.”
