Manage your palliative care journey with MyChart
Access your care team, appointments, and health notes at any time.
How MyChart helps patients and caregivers
MyChart is a secure online tool that helps you manage your care in one place. For patients receiving palliative care and their caregivers, the MyChart app makes it easier to stay informed, organized, and connected with your Carelon Palliative Care team.
New federal rules, like the 21st Century Cures Act , let you access your health information faster. You may see your test results, doctor notes, and care plans in MyChart — often as soon as they are ready. Having quick access to this information can help you feel more in control and better prepared to make care decisions.
With MyChart, you can:
- View test results and visit notes.
- Message your care team anytime.
- Keep track of medications and appointments.
- Share updates with family members and caregivers.
- Manage your care from your smartphone, tablet, or computer.
Using the MyChart app can help reduce stress, save time, and make it easier to understand your care. Whether you are a patient or a caregiver, MyChart gives you simple tools to support your palliative care journey with Carelon.
