Why you may be hearing from Carelon about palliative care

You may have received a call or referral through your health plan. Your plan works with Carelon to identify members who could benefit from additional palliative care services to better manage symptoms and coordinate care.

Carelon partners with your health plan to provide this support as part of your covered benefits. It is designed to support you, not replace your current care. Participation is always your choice.

How Carelon supports you day to day

Palliative care services provide extra support for people living with serious illness by helping to manage symptoms, reduce stress, and improve quality of life.

These services are provided alongside your treatment and are often delivered at home:

  • Your care team responds quickly and is available 24/7 when you need support
  • Help managing symptoms like pain, fatigue, or nausea
  • Coordination across your doctors and specialists
  • Support for caregivers and family members
  • Guidance when making care decisions

Our goal is to make your care easier, more coordinated, and focused on what matters most to you.
